Improve patient outcomes and expand access to care for underserved populations with remote patient monitoring (RPM)
Using RPM, community health centers (FQHCs, RHCs, and CHCs) can identify and address potential health problems early on, preventing complications and exacerbations.
Remote patient monitoring is an effective way to help you manage chronic care patients and address a lot of the gaps that are experienced as SDoH barriers. Empower your patients with the latest remote care technology from HRS.

Priority Cellular Network Access through the FirstNet First Responder Network
By having multiple ways of delivering RPM services, HRS allows us to provide that capability to patients of all different aspects who may not have Wi-Fi at home, who may not have a smartphone at home. The RPM kits have really been a tremendous value to many of our patients. We're able to provide that to a group of patients who we might not be able to provide access to any other way.
Not every patient needs a complete remote patient monitoring kit to manage their health condition. That's why our solution is flexible to meet the needs of every organization and patient. As an HRS client, you'll have the full support of our clinical, reimbursement, onboarding, integration, and technical support teams to design an effective, sustainable RPM program.
