Dr. Ghazwan Toma is an Assistant Professor at the Department of Family Medicine of University of Michigan. Toma is the Medical Director of Patient Monitoring at Home of Michigan Medicine.
Dr. Toma earned his medical degree from Baghdad University (2001) and completed a Master’s of Public Health from the University of Albany (2007). Toma completed his residency training in Family Medicine at Beaumont Hospital (2011) and a fellowship training in Geriatric Medicine at the University of Michigan (2012). Dr. Toma is passionate about chronic diseases management, preventive medicine, and complex care.

Prior to HRS, Jenna Kowalski worked for 17 years as an RN at a large health system in western New York. Jenna started her career in the acute step-down unit and the ICU, later moving to home health where she held many positions including Lead Cardiac Trainer, Clinical Educator, Manager of Clinical Operations, and most recently, the Director of Nursing. She holds a Bachelor’s degree in nursing and an MBA with an advanced certificate in healthcare management.
Remote patient monitoring and telehealth technologies have been increasingly deployed in post-acute care settings to extend care services to the home, prevent unnecessary readmissions and hospitalizations, and reduce nursing burden.
