CHI Health at Home has partnered with Health Recovery Solutions to pilot a unique patient engagement platform.
MILFORD, Ohio-- CHI Health at Home, one of the largest healthcare systems in the country, partnered with innovative software company, Health Recovery Solutions (HRS) in December 2017 to pilot a cutting edge remote monitoring platform designed to influence patient behaviors while reducing rehospitalization. CHI Health at Home will use the HRS platform to continue to support its mission in delivering quality, compassionate care to every patient they serve.
Beginning in Kentucky and Nebraska, CHI Health at Home will be the first provider in these two states to partner with national telehealth leader, HRS, to reduce hospital readmissions for chronic disease patients and improve overall quality of care. Patients with congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD) will have the opportunity to receive telemonitoring to support their at-home chronic care management and reduce the risk of hospital readmission and ER utilization.
Patients placed on telehealth will be provided with 4G enabled tablets pre-loaded with HRS' disease specific software paired with Bluetooth devices that allows for biometric monitoring such as blood pressure, weight, oxygen saturation levels, and more. Patients will monitor their vitals daily and answer daily symptom questions specific to their disease condition. All data captured through the devices will automatically and securely transmit back to the CHI Health at Home's clinical team within seconds, allowing for real-time interventions for their high-risk patients.
The HRS software was specifically designed to address the primary drivers of hospital readmissions and ER utilization rates of chronic disease patients, including medication non-adherence, low health literacy, and lack of communication with clinicians and caregivers. Moreover, the HRS software has a bi-directional integration with their electronic medical record (EMR), allowing for continuous patient care across patient care teams. In addition to the software, CHI Home Health will be using HRS PatientDirect which provides all logistics and inventory management of telehealth units. HRS PatientDirect ensures that telehealth units are delivered to the patient's home and also picked up at the end of care, creating a seamless experience for patients.
